Welcome to our 30-minute follow-up visit for ongoing health coaching and navigation. Our team of experts will provide you with additional education, analyze your medical records, investigate your health concerns, prior to the visit. We will also provide recommendations for moving forward, based on your personal desires, including a visit summary outlining recommendations, emailed to you after the visit. We are committed to helping you achieve your health goals and look forward to working you.

15 minutes visit- $40
